This section asks questions about specific checks your employer has requested as part of your screening.  It is your chance to declare anything adverse that may arise during your screening.  Specific guidance over the information you will need to declare is given below, but as a rough guide, if you are in doubt, it is better to declare information than not to.  If after reading the guidance below, you are still unsure whether or not to make a declaration, please speak to your recruiter or HR contact with your employer.

Please answer Yes or No and give full details of any offences should your answer be Yes. Please note that offences such as driving under the influence of alcohol are also criminal convictions and as such should be declared in this section.

Please tell us about any proceedings currently underway against you: for example if you have been charged with a crime (including driving under the influence of alcohol and similar offences) but have yet to be found guilty or not guilty.

The information you supply will be checked against consumer database sources, but this does not constitute a 'credit check'.  We are not checking for approval for credit but for publicly held adverse information held about you.

Have you ever had a County Court Judgement (CCJ), Individual Voluntary Arrangement (IVA), Debt Management Plan, Trust Deed or other judgement debt, whether satisfied or not and whether discharged or not, or are you aware of any such action pending? *

Please give us full details of any of the types of financial information held about you as detailed in the question.  If you are not sure if any of these judgments have been passed against you, you may wish to contact a credit checking agency and run a check against yourself.  These checks are usually inexpensive, available online and carried out instantly.  An internet search for ‘credit checking agency’ will give a range of websites that offer this service.

Please tell us about any companies you have been a director of in the parameters shown.  Please tell us the name of the company, dates of trading, nature of business and your current status with the company.

Please tell us details of any employments you have had that ended due to dismissal, including the name of the company, job title and the circumstances in which you left.  Please note that if you have declared ‘dismissed’ as a reason for leaving any of the employments you have declared in the Employment History section then you should detail the circumstances here.

Please tell us about any live or pending disciplinary warnings or hearings against you, including the details why, dates and the current situation with your employers: for example, you may be waiting for a disciplinary hearing shortly.

Driving licence information (if applicable)

Do you hold a driving licence?

Please let us know if you have a driving licence.  If not, you do not need to answer any further questions in this section.  Your employer will have indicated that you are driving as part of your role or that you have a company car as part of your employment, which is why they have asked us to check the conditions of your driving licence.

Please tell us what type of licence you hold: for example, a provisional licence, HGV licence, international driver’s licence.

Please give us your driving licence number as it appears on your licence.  In the UK this is displayed on your photo card as item 5.

Date of expiry

Please tell us the country your driving licence was issued in.

Please detail any penalties (if applicable)

Please give details of any driving penalties on your licence.  This includes fixed penalty notices for speeding or parking offences, and driving related criminal convictions, including driving under the influence of alcohol.
